Report a Concern

Share important concerns with FLCC securely and confidentially. Report incidents to help our community receive the support they need.
This reporting process is not intended to report life-threatening emergencies. If you or the individual you are concerned about requires emergency services, call 911 or FLCC Campus Police at 585-785-1900.

When to Report

FLCC is committed to providing a safe, supportive community for all students, faculty, staff, and visitors. If you experience or witness something concerning, we encourage you to report it.

Creating a report enables us to address incidents within our community to provide prompt support for those in distress. 

You should submit a concern when you observe behavior or an incident that worries you, seems unsafe, or potentially violates college policies.

Some examples include:

  • Concerning, threatening, or disruptive behavior
  • Bias, harassment, or discrimination
  • Suspected or known substance abuse
  • Sexual misconduct (Title IX)
  • General worries about a student, employee, or community member

In short, if something doesn’t feel right or poses a risk to someone’s well-being, it’s worth reporting.

How to Submit a Report

Use our secure Report a Concern form to file your report. This single form allows you to report any concern, whether it’s related to student conduct, safety, bias, or personal well-being, for any community member. Descriptions for each form are provided.

You’ll be asked to provide information such as who was involved, what happened, when and where it occurred, and why you’re concerned. Each report is routed to the right intervention team.

Report a Concern

You can remain anonymous when submitting the form. While we encourage you to include your contact information, anonymous reports will still be reviewed and, if possible, addressed.

Providing your details helps us follow up for clarifications and keep you informed, which can assist in resolving the concern.

All reports are handled confidentially, with information shared only on a need-to-know basis.

    • Report about an employee: Forwarded to Human Resources to determine an appropriate response. 
    • Report about a student: Reviewed by the Behavioral Assessment Team to determine the urgency of the situation. 
    • If the situation is deemed urgent or concerning, the BAT will convene and speak to relevant stakeholders to determine the best way to assist and keep the community safe. 
    • Situations not deemed urgent are assigned to individuals within Student Affairs, depending on the situation. A student is not required to respond to outreach. 

    Note: If the reporter is not directly involved as the complainant or respondent, they may not receive any additional information due to FERPA or personnel systems.

Behavioral Assessment Team

The Behavior Assessment Team (BAT) is FLCC’s multidisciplinary group dedicated to keeping our campus safe and supporting students who may be struggling. The BAT brings together professionals from Student Affairs, Counseling Services, Campus Police, and other areas to review reports of threatening or concerning behavior and coordinate appropriate interventions.

The following individuals serve on the Behavior Assessment Team:

  • Associate Provost of Student Affairs
  • Campus Police Chief, Assistant Police Chief
  • Director of Counseling
  • Director of Housing
  • Director of Student Success
  • Disability Services Manager
  • Human Resources RE
  • Senior Director, Center for Student Well-Being

Fri: 8:30 a.m. – 4:00 p.m.
